We provide the services listed below throughout
the NYC-METRO area, inclusive of Westchester County. Other services may be arranged, per client request.
Fees are subject to change. Regional/long-distance telephone charges and transportation reimbursement for on-site visits are
billed additionally. All fees are payable in full at the time of service delivery.
All clients will receive a NYCNHCS contract, and HIPAA 'Privacy Practices' Notification/Consent Forms prior
to service delivery. Clients may pay for services by cash; personal or certified check or money
order.
