Medical And Financial Services Offered In The New York Area 
 
We provide the  services listed below throughout the NYC-Metro area.  Other services may be arranged, per client request.  Fees are subject to change.  Regional/long-distance telephone charges and transportation reimbursement for on-site visits are billed additionally.  All fees are payable in full at the time of service delivery. 
 
All clients will receive a NYCNHCS contract, and  HIPPA 'Privacy Practices' Notification/Consent Forms prior to service delivery.  Clients may pay for services by cash; personal or certified check or money order.

 
 
Initial Telephone/Email Consultation: $60. 
 
Subsequent Telephone Consultations: $60/hr.
 
Home/On-Site Consultations & RN Assessments (Manhattan/Bronx/Brooklyn/
Queens/Westchester/Rockland): $100/hr.
 
Service Arrangement & Coordination: $100/hr.
 
RN Case Management: $75/hr.
In-Home Medical Assessments

 
 
Skilled-Nursing/Assisted Living Facility Placement: $350 (Per Person/All Inclusive).
 
Dementia Care Placement: $350 (Per Person/All Inclusive).
 
Short-Term/Sub-Acute Rehabilitation Placement: $350 (Per Person/All Inclusive).
 
Home Care Services Arrangement: $75 (Per Person/All Inclusive).
 
 
Quality Skilled-Nursing & SubAcute Facilities

Escorted Facility Tours
Escorted Facility Tours: $100/hr.
 
PRI  Form Completion For Skilled Nursing Facility Admission: $75.
 
DMS1 Form Completion for Home Care Services Arrangement: $75.
 
Research: $75/hr.
 
Written Emailed/Mailed/Faxed Reports: $60/report.

Financial Planning Services
Financial Planning Services
Asset Planning Consultation: $75/hr.
 
NY Medicaid Application: $75.
 
Adult Social Security Disability Application: $75.
 
NYSARC Trust Application: $300.
